Social Worker MSW - Home Health King County
The Medical Social Worker is responsible for providing services to clients with psychosocial needs affecting the delivery of agency care of the potential outcome or response to care provided. The MSW develops and implements a plan of care in collaboration with the physician and other members of the interdisciplinary home care team.

Master’s Degree in Social Work from an accredited school of social work
Current Social Worker Certification or Licensure through Washington State Department of Health
Valid Washington State driver’s license
Current Healthcare Provider CPR Certification
Access to daily use of an insured reliable vehicle or an alternate ability to travel to sites
Certified Case Manager (CCM)
Prior experience in Medicare Certified Home Care Agency
2 years experience in a healthcare setting
