The Activities Planner I Social Services Designee is responsible for overseeing and leading the Activity Programs & Social Service Support as required by Title 22 and Medicare Guidelines.
Required qualifications:
Activities Designee Training from an approved program.
Social Services Designee Training from an approved program.
Activities Designee Certification from an approved program upon hire.
Social Services Designee Certification from an approved program upon hire.
Consultation by an approved LCSW
2 years Experience in the post acute setting (SNF).
