We are seeking Quality Specialist who will function as a facilitator to the organization-at-large and to the Medical Staff in their commitment to improving the quality of patient care and organizational performance through the Performance Improvement Program. This individual abstract and manage appropriate data for a variety of purposes including indicator reporting projects, information management and decision making, and tracking and trending data for identification of variations in the delivery of patient care. Work with the physicians and hospital personnel to develop projects for review and analysis of aggregated data. Assist the Director, Performance Improvement in the planning, development, implementation and management of the hospital-wide quality improvement program which is in compliance with JCAHO and other regulatory requirements. In conjunction with the department managers, medical staff and the Care Council, monitors the program's effectiveness in improving the quality of patient care and services rendered

In this position you will:

  • Review / update indicators and criteria to reflect changes in standards of practice, benchmarking goals, etc

  • Coordinate department review of medical staff cases to the various medical staff departments.

  • Trend all Medical Staff activities as defined by the peer review process and PI activities.

  • Notify responsible physician of department cases in format of request to attend / present cases as soon as case is targeted for department review.

  • Screen cases and reviews based on established indicators

  • Facilitate presentation of cases in department meetings by arranging for the medical chart to be available for review by the responsible physician prior to the meeting and at the meeting, records physician comments and then places same in case review system.

  • Attend Medical Staff and Hospital meetings as necessary and presents data and/or information related to PI activities

  • Assure periodic review of PI plans for the medical staff and annually evaluates the effectiveness of the Medical Staff’s PI plan.

  • Provide timely reports to the Medical Staff departments in accordance with their specific PI plans.

  • Coordinate the information and department activities and sends package to the Director to include in PI agenda at Leadership Council (LCMH-Torrance / LCM-SPH MEC).

  • Review inpatient mortality cases as appropriate.

  • Review all blood usage cases referred from the blood bank.

  • Review all cases referred to QM including cases referred from HIS.

  • Assist in the credentialing process for the Medical Staff through accurate and timely reporting of all reviews done for the specific physician.

  • Work with department managers and PI groups through orientation, on-going education and participation in PI groups and committees.

  • Develop, report and present data in a detailed fashion to various medical staff committees, management, and employee groups related to areas of expertise.

  • Develop and support effective communication and coordination between PI department and Medical Staff Office, as related to PI activities.

  • Serve as the consultant for the PI functions and peer review / case review functions throughout assigned continuum.

  • Documentation / Information management

  • Knowledgeable in JCAHO standards for Performance Improvement and Leadership and Medical Staff leadership and keeps current with revisions. Maintains the confidentiality of all patient documents / database information

  • Knowledge of databases in use in the PI function.

  • Utilize hospital information systems whenever possible to retrieve data and/or assist in the trending of required information

  • Provide reports to the Care Council and Leadership Council (Torrance) related to PI activities and data being trended and/or required for review. Enters data / information into the Case Review system.

  • Work in conjunction with Medical Staff Office to prepare reports for PI related activities.

  • Utilize Microsoft Word, Excel and Access in a proficient manner.

  • Leadership / Supervision:

  • Maintain current job description and provides a summary of accomplishments annually.

  • Lead the performance improvement and quality functions.

  • Work closely with PI staff to ensure smooth operations of the PI functions.

Qualifications:

Required qualifications for this position include:

  • Associates degree in a health-related field

  • knowledge of JCAHO standards and/or regulatory agency requirements regarding PI/Quality

  • Must be organized, detail oriented and like to work with statistics and evaluation of methodologies

  • Ability to deal effectively with all levels of hospital personnel, the public and physicians

  • Computer literate

Preferred qualifications for this position include:

  • Bachelor's Degree

  • Current California State Registered Nurse License

  • CPHQ certification

  • Coursework/Training: Blakbelth Certification

  • Two (2) performance improvement or related area.
