Providence LVN - Hospice Crisis Care - SIGN-ON BONUS in Torrance, California

Providence is calling a Licensed Vocational Nurse (LVN) for the Crisis Care team (full-time/variable shift) to Providence TrinityCare Hospice.

The schedule for this position is primarily Mon-Fri, 8:00am-4:30pm, with some shifts at 4:00p-12:30am or 12:00am-8:30am. It requires working 2 weekend days a month, or one full weekend per month.

We are seeking a Licensed Vocational Nurse (LVN) who will be responsible for providing direct patient/family care under the supervision of a Registered Nurse for the Hospice Care Program. Responsibilities include following the Plan of Care, providing treatments, and working collaboratively with the members of the team to help meet positive patient/family care outcomes. May provide care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.

In this position you will:

  • Provide direct patient care as defined in the State of California Nurse Practice Act

  • Implement current nursing practice following a patient assessment and an approved Plan of Care initiated by the Registered Nurse

  • Provide patient/family/caregiver education and information pertinent to diagnosis and Plan of Care

  • Participate in coordination of home-based palliative care services, appropriately reporting the identified needs for the Interdisciplinary Group (IDG) members, e.g., Certified Home Health Aide (CHHA), OT, PT, MSW, ST, Dietitian, Chaplain to the clinician or Patient Care Manager

  • Provide appropriate pain/symptom management

  • Evaluate patient’s response to treatments/medications

  • Understand the hospice benefit and levels of care

  • Maintain productivity standards as set by organization guidelines

  • Use equipment and supplies effectively and efficiently

  • Documentation / Information Management:

  • Provide accurate and timely documentation of patient services to reflect the Plan of Care

  • Submit documentation for the patient medical record within the timeframe set by the organization guidelines

  • Leadership / Supervision:

  • Participate in orientation, education, and development of team members as directed by supervisor

  • Development:

  • Participate in appropriate continuing education as may be requested and/or required by immediate supervisor

  • Accept personal responsibility for other educational activities to enhance job-related skills and abilities

  • Performance Improvement:

  • Participate in the Performance Improvement Program and participate in and adheres to the PH&S/TrinityCare Hospice Corporate Integrity Plan

  • Budgetary:

Qualifications:

Required qualifications for this position includes:

  • Graduate of a Vocational Nursing program

  • Currently licensed as an LVN in the state of California

  • Possesses and maintains current BLS certification

  • Must be a licensed driver with an automobile that is in good working order and insured in accordance with state and/or PH&S/TrinityCare Hospice requirements

  • Minimum 1-year professional Nursing experience

  • Must obtain IV certification within the 6 months of hire

  • Comply with accepted professional standards and practice

  • Demonstrate good verbal and written communication and organizational skills

  • Demonstrate understanding of hospice philosophy and concept and understands the needs of the terminally ill

Preferred qualifications for this position include:

  • Community health/hospice/palliative care or medical/surgical experience.
