Providence is calling a Registered Nurse-- Cardiology RN to be our Care Specialist - Providence Adult &Teen Congenital Heart Program (PATCH) (Congenital Heart Disease, Day shift, 1.0 FTE, full time, at Providence Medical Group in Spokane, WA. This position is located on the 4th floor of Sacred Heart Children's Hospital.

The primary role of the ACHD Nurse Coordinator is to participate in the moderate and higher level care of the teen and adult patient (ages 16 years and older) and serve as the primary point of contact for PATCH patients in need of medical advice, medicine titration, and triaging of care. His/her goal is to ensure that PATCH patients feel that they are cared for by a team of providers and have their most important health needs addressed. The ACHD Nurse Coordinator serves as the intermediary between the patient and the ACHD APP/MD for all issues that fall outside of the scope of the ACHD MA, such as conveying of abnormal test results, patient calls for symptoms that require triaging, arrangement of complicated treatment plans, etc. The ACHD Nurse Coordinator will also organize and oversee the ACHD education of inpatient and outpatient MAs, LPNs, and RNs, a role not limited to organizing/leading of the quarterly ACHD RN Education Conference and bi-annual ACHD OB RN Education Conference. Finally, the ACHD Nurse Coordinator will be involved in clinical and non-administrative aspects of the PATCH program and its development and growth, particularly as it pertains to implementation of clinic policies/protocols that improve the patient experience.

In this position you will:

  • Provide leadership and continuity of care guidance for patients with chronic diseases and/or complex disease states.

  • Use knowledge of illness, diagnoses and treatments in comprehensive, holistic assessments of patients.

  • Provide teaching to patients/families related to patient’s diagnosis, pathology, medical and nursing treatment plans, discharge needs and health goals; documents each element of care per organizational and unit standards.

  • Utilize appropriate resources to gather data and assess patient care needs; make ongoing assessments of the patient including but not limited to: physio-biological, cognitive/emotional, spiritual and social, managing life with chronic disease.

  • Identify possible problems that could lead to an emergency/crisis situation and takes appropriate action to de-escalate the potential for the situation to occur.

  • Maintain complete documentation of all critical and essential aspects of medical history and information in the patient’s file. Manage patient related activities in an outpatient, ambulatory clinic and/or inpatient, acute care setting.

  • Assist in designing and implementing planned care interventions through a combination of evidence-based guideline/pathway utilization and individualized care planning.

  • Mobilize resources and manages the system in response to patient need according to an interdisciplinary plan of care.

Qualifications:

Required qualifications:

  • Graduate from an approved, accredited school of nursing

  • Currently licensed as a Registered Nurse (RN), with the ability to obtain a WA state RN license in order to practice in Washington state

  • Cardiopulmonary Resuscitation (CPR) certification within ninety (90) days of hire

  • Two (2) to five (5) years of work experience in an RN role

Preferred qualifications:

  • Cardiology nursing experience highly preferred

  • Congenital heart nursing experience preferred

  • Experience managing complex patients in an ambulatory setting
