Providence Health & Services RN Case Manager in Seattle, Washington

Providence is calling a Registered Nurse (RN) Case Manager (PACE Full Life Dept, Day Shift, 1.0 FTE, 40 hours/week Monday-Friday) to Providence ElderPlace in Kent, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.

In this position you will:

  • Facilitate interdisciplinary team care coordination, intensive medical management of participants recently discharged from hospital and skilled nursing facilities.

  • Collect the necessary medical and social information to ensure comprehensive decisions are made which ensure the safety and well being of participants.

  • Act as a medical advocate for the participant, and coordinate care services with the Interdisciplinary team.

  • Build strong relations with participants, caregivers, families and community providers so that communication and coordination occurs in a manner that facilitates optimal care and financial oversight.

Qualifications:

Required qualifications for this position include:

  • Graduate of accredited school of nursing

  • Current Washington State Registered Nurse (RN) license

  • Current Cardiopulmonary Resuscitation (CPR) certification

  • Current CCM, NACCM or ACME upon hire or must be obtained within 1 year of employment

  • Current Washington State drivers license

  • Proof of auto insurance

  • 1 year of long term care or geriatric nursing experience

Preferred qualifications for this position include:

  • Bachelor Nursing Degree

  • Medical Surgical Experience or Rehabilitation Medicine
