Providence Health & Services LPN in Seattle, Washington
Providence is calling a Licensed Practical Nurse (LPN) (Evening Shift, 1.0 FTE) to Providence Heritage House at the Market in downtown Seattle, Washington. In this position you will:
Assist nursing staff in planning and compliance with medication regime for each participant.
Administer care based on the nursing process, the interdisciplinary care plan and the nursing care plan.
Provide direct participant care, including but not limited to: vital signs, wound care, medication administration/management, treatments, patient teaching, venipuncture and microcoagulation, catheter care foot care, administration/reading PPD tests and patient set up for physician visit.
Contribute to the total care plan by making pertinent nursing observations and participating in multidisciplinary team care conferences, staff meetings and in-services as needed.
Assure that proper equipment, instruments and supplies are available to meet patient needs.
Assure that documentation reflects adherence to nursing standards, interdisciplinary care plan and progress toward identified patient outcomes.
Act as a liaison between participant, family/guardian, physicians, RN’s, Nurse Manager and Administration as needed or delegated.
Required qualifications for this position include:
Graduate of an accredited Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) program
Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) licensure in Washington State
Current Cardiopulmonary Resuscitation (CPR) Certification
1 year long term care or related geriatric experience
Experience working with the frail and elderly
