Providence is calling a Licensed Practical Nurse (LPN) (Float Pool, Variables, 0.0 FTE) to Providence Mount St. Vincent in Seattle, Washington.
This position provides professional nursing services to residents using a resident-directed social model. Primary functions are assessment of resident health status and service needs, acting as a consultant for residents and families regarding health issues and medications, and limited direct nursing services as defined by program policies.

In this position you will:
Understands, articulates and practices a resident-directed social model of care for assisted living and provides health care services according to this model.
Participates in the pre-move health assessment of potential residents, including a review of the history and physical paperwork, and meeting with the resident and family/friends during the final interview for a face-to-face assessment and initial service planning for health services.
Using a team approach, determines appropriate medication service category(s) for residents both at the time of move-in and on an ongoing basis.
Assists medication coordinator with Medication Assistance services including medication log in, labeling of cards and bags, maintenance of med supply for resident assistants, entering orders into EMR, setting up MARs for resident assistance, management of back-up supply and ordering meds when needed.
Administers medications per physician order, including maintenance of accurate records for medication administration.
Required qualifications for this position include:
Associate's Degree in LPN
Two (2) years post-license nursing experience
Current Washington State nursing license
Current CPR certification or able to obtain within 30 days of hire
Hand in Hand Assisted Living staff: Certificate of completion of Dementia and Mental Health specialty training or able to obtain within 90 days of hire
Supportive Care SJR staff: Nurse Delegation Certificate
