Providence Health & Services Inpatient Case Management Coordinator (RN) in Seattle, Washington
Providence is calling a Inpatient Case Management Coordinator (RN) (Hospital Administration, Day Shift, 1.0 FTE) to Providence ElderPlace in Seattle, Washington. In this position you will:
Monitor ElderPlace Administrative System (EPAS) desk top and call sheets to be apprised of PEP participants admitted to the ED and/or hospital inpatient service.
Authority for health plan decisions related to areas such as length of stay
Collects all pertinent information from Centricity regarding medical condition, care plan, recent diagnostics and participant POLST and health care wishes.
Assures pertinent information is communicated to ED physician, hospitalist, or other key staff and documented in the hospital chart.
Communicates status and treatment goals to PEP primary care provider.
Communicates daily with primary care providers and the interdisciplinary team about status of hospitalized participants; makes rounds as needed to assess status, progress and discharge needs; makes clinical follow-up arrangements.
Makes rounds as needed to assess status, progress and discharge potential assuring communication flow between hospitalist, PCP and IDT.
Gives report to IDT daily in addition to the PCP.
Coordinates with hospital nursing and discharge planning staff for discharge related information.
Maintains log of members in acute and short-term SNF units including dates of admission
Coordinates discharges from hospital to home and hospital to nursing home working in collaboration with PEP team and other health care providers as appropriate.
Communicates with nursing home staff nurses or other care givers all nursing and medical related information; informs Transportation Dept. about transport needs of participant upon discharge.
Required qualifications for this position include:
Current, unencumbered Washington Nursing License required
Two (2) years Medical/surgical experience.
Preferred qualifications for this position include:
Recent ElderPlace, hospice, home health, utilization or care management experience.
Recent acute care or long term care experience.
