Social Work Care Manager - in Management Department at Providence Seaside Hospital located on the beautiful coast of Seaside, Oregon. This is a full-time 1.0 FTE Day position.
$10,000 Hiring Bonus for eligible external candidates who meet all conditions for payment - this is in addition to the fantastic benefits and compensation package offered by Providence that begin on your first day of employment.

The Care Management (CM) Manager is accountable for the direction and management of the inpatient and PMG Care Management Departments, as well as supporting Home Health and the Palliative Care program for the North Coast. This position provides leadership and oversight for the clinical care management model, and operations of the care management department for the facility and PMG medical clinics.
Required Qualifications:
Master's Degree in Social Work
Oregon Registered Nurse License
5 years Supervisory experience, including program planning, performance management, staff development and evaluation
5 years Care Management experience
