Providence is calling a Physical Therapist - Outpatient Orthopedics and Neurology (1.0 FTE, Days) to Providence Gearhart Outpatient Clinic in Gearhart, OR. This position will primarily work in an outpatient setting, but we are providing the opportunity to be trained in an inpatient setting and to work some of your role in Providence Seaside Hospital as well! This position's shift can be during weekdays and weekends starting as early as 7:30am and ending as late as 6:30pm.

Seaside hospital has a flexible and team-oriented environment. If you are looking to break into any specialty of physical therapy, whether it be neurology, pelvic health, vestibular, concussion, or others, we are willing to mentor you and help you achieve that goal. We are also willing to work with schedules that work best for you, whether that is working 4 10s, 5 8s, or some days shorter and longer.

We are seeking a Physical Therapist - Outpatient Orthopedics and Neurology that will manage and provide skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities, taking into account the individual patient special physical or age-related needs. Have the skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individual functional level and potential.

In this position you will have the following responsibilities:

  • Provide appropriate, timely and effective evaluation and re-evaluation of patients:

  • Evaluate patient/client current physical status and develops individualized, measurable and functional treatment goals and establishes interventions as appropriate to patient age, condition, diagnosis, response to treatment and learning needs

  • Incorporate patient and family as appropriate in goal setting

  • Re-evaluate and update goals in a timely manner

  • Plan and implement a comprehensive and individualized physical therapy treatment plan:

  • Modify and progress the program in response to changing patient/client needs

  • Instruct patient/client and caregiver to assist in achieving treatment goals

  • Terminate treatment as appropriate in relation to patient/client’s age, condition, diagnosis and functional performance / outcome expectations and discharge criteria

  • Communicate frequently and effectively in the professional environment with patients, caregivers and others:

  • Provide and maintain accurate, thorough, legible and timely patient related documentation, handwritten and/or computerized, in accordance with department policy and procedure

  • Communicate frequently and effectively with others involved in patient/client’s care regarding scheduling, treatment plans, progress and discharge planning

  • Provide non-patient related documentation accurately and in a timely manner to Manager /Lead

  • Maintain positive working relationships

  • Demonstrate professional behavior and pursues professional development:

  • Maintain and enhance personal expertise through appropriate professional educational experiences

  • Maintain required hospital and department competency in environment of care, age-related care, CPR and other required system trainings

  • Maintain a high standard of professional accountability with respect to appearance, demeanor, safety, ethical and legal issues

  • Provide a comfortable, orderly, clean, and safe work environment

  • Keep informed of health care issues, changing department procedures, and other department information by attending staff/team meetings and/or reading staff meeting minutes, e-mails and newsletters

  • Enhance personal professional growth by establishing individual goals, providing in-services to others, participating in community activities, and contributing to program development

Required qualifications for this position include:

  • Bachelor's, Master's, or Doctorate degree from an accredited Physical Therapy Program

  • Current Physical Therapy Licensure through the State of Oregon, -OR- Temporary license if new graduate awaiting licensing exam results

  • American Heart Association Basic Life Support (AHA-BLS) with Automated External Defibrillator (AED) for Healthcare Professionals within 30 days of hire

  • National Provider Identifier (NPI)
