Location: Santa Rosa, CA
Work Schedule: Full Time - 80 Biweekly Hours
Shift: 8-hour, Days

The Hospice Care Manager maintains an assigned patient/family caseload from admission through the patient's death. Develops and updates the patient/family Plan of Care working collaboratively with the members of Interdisciplinary Team (IDT). Identifies patient/family care needs, implementing nursing interventions as appropriate. Evaluates the results of interventions and adjusts the Plan of Care accordingly. Demonstrates knowledge of costs related to patient care services and manages according to department guidelines. Communicates changes in patient/family condition to the Patient Care Coordinator and team members in a timely manner. Understands the details of insurance reimbursement for nursing services.

Current BLS (Basic Life Support for Health Care Provider) or ACLS (Advanced Cardiac Life Support) required within 30 days of hire
Current CA RN license
Valid driver's license & insurance
One (1) year experience as a RN

About the Team
Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life. Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.
