We are seeking a Director Pharmacy who will function in a team leadership role to assure delivery of client continuous quality improvement. Line duties include consultative services to medical, nursing, and pharmacy staff. Provides prescribed medications, drugs, and other pharmaceutical, and compounds medication, according to professional standards and practices. Plans, organizes, directs and evaluates Hospital Pharmacy program.
In this position you will:
Perform assessment/data collection.
Ensure compliance with regulatory agencies governing health care delivery.
Perform on-going assessment of patients and patient care/department area.
Formulate goal directed plan based on patient/department outcomes.
Initiates, individualizes, and modifies plan of care.
Collaborate with other services.
Implement role based on department and hospital standards.
Follow established protocols and procedures.
Evaluate effectiveness of duties/department.
Identify patient/significant other learning needs and demonstrates measures to meet these.
Delegate to appropriate patient care team members.
Demonstrate applied knowledge when caring for selected patient populations including care appropriate to the age of patients served on unit.
Demonstrate competency in selected skills including, but not limited to:
Receive, verify, and fill all types of medication orders including, TPN, antimicrobial, chemotherapy, Colony Stimulating Factors and Epogen.
Educate staff on medication actions, indications, and side effects.
Check, restock, and purchas inventory in accordance with hospital needs.
Conduct Drug Utilization Reviews and prepares Drug Review Criteria for review.
Demonstrate responsibility and accountability for own professional practice in regards to:
Attendance, punctuality, and confidentiality.
Positive public relation image and working relationships.
Obtaining necessary knowledge and skill needed to maintain competence.
Completing all mandatory requirements, including annual safety films, CPR, infection control and annual physical.
Participate in the development and attainment of team, department and hospital goals including:
Attending and participating in team meetings.
Volunteering or accepting assignment for projects.
Mission, vision and team ground rules.
Demonstrate documentation on department records and forms.
Maintain department reports and records.
Collect statistical data for administrative and regulatory purposes.
Control narcotics/controlled substances.
Demonstrate Director skills including but not limited to:
Develop and corroboratively implements budget strategic plans, programs, and policies.
Effectively manages team/department resources.
Facilitate employee and team development.
Ensure high quality team performance in the following areas:
Maintenance of patients' rights and responsibilities
Quality improvement programs
Safety, environmental and infection control standards
Compliance with all regulatory agencies governing health care delivery and the rules of accrediting bodies
Required qualifications for this position include:
Graduate of a School of Pharmacy recognized by the California State Board of Pharmacy.
Current California Pharmacist License.
3 years Experience in director position.
2 years Hospital practice emphasizing clinical pharmacy, pharmacokinetics, and drug information services, or must have completed a clinical pharmacy residency.
Ability to deal effectively with many people.
Ability to prioritize, organize and coordinate work.
Ability to apply sound judgment.
Knowledge of budgeting, CQI, teaching, computer use, drug packagings and preparation equipment.
Good oral and written communication skills must be demonstrated.
Preferred qualifications for this position include:
Master's Degree in Sciences.
