have the opportunity to provide one to one compassionate care in the comfort of
the patient's home. This unique setting allows for our direct patient care
staff to provide creative individualized care for the patients we serve.

duties for this role includes catheter care, wound care, disease management
teaching and medication management under
the direction of a nurse case manager or clinical supervisor following the
established nursing process and procedures, in accordance with the Nurse
Practice Act and in compliance with all applicable State and Federal
regulations and in compliance with all regulatory bodies. The schedule for this
position 8:30am-5:00pm and provides home care within and surrounding
communities of South Salem, Lebanon, Dallas, Detroit, and Corvallis.
Required qualifications for this position include:
Current, unencumbered Oregon Registered Nurse (RN) license
Current American Heart Association Basic Life Support (AHA BLS) for Healthcare Provider certification
Valid driver's license and reliable transportation
Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire
2 years of experience in acute care setting
In compliance with the Department of Human Services (DHS), a criminal background check is required for this position
Preferred qualifications for this position include:
- 2 years experience in Home Health - or - Public Health nursing
