Providence Population Health Clinical Coordinator in Renton, Washington
Providence St. Joseph Health is calling a Population Health Clinical Coordinator (Full-Time, Days) to our location in Renton, WA.
We are seeking a Population Health Clinical Coordinator who will be responsible for coordinating multiple aspects of care for patients with preventative care needs, chronic diseases, and care opportunities within the Physician Enterprise. Daily work includes ordering laboratory tests or imaging exams per protocol to facilitate patient screenings, management of preventative care, and supporting optimization of chronic disease treatments as prioritized thru Enterprise Clinical governance. The Population Health Clinical Coordinator assures that an integrated approach to the patients’ care coordination is fulfilled. This position is the liaison between Physician Services Enterprise providers, primary care clinics, specialist offices, patients, multiple Providence St Joseph Health scheduling and ancillary departments (e.g., Laboratory), and freestanding care delivery centers or services for population health programs and related processes. Performs all duties in a manner that promotes team concept and reflects the Sisters of Providence mission and philosophy.
In this position you will have the following responsibilities:
Triages clinical disease registries in Epic and prioritizes coordination of patient’s chronic and preventative care needs.
Acts as a resource and contact for the Physician Enterprise Population Health Program and provides technical assistance to providers, clinical support staff, and/or Clinical Pharmacy Specialists. Serves as a liaison and resource to the clinic staff and the patient for optimizing the coordination of care and immediate problem resolution. Collaborates with other team members to ensure the best patient outcomes.
Orders laboratory tests or imaging exams per protocol to close care gaps.
Requests and abstracts clinical data (eg. immunizations, laboratory tests, imaging studies, procedures) in Epic from external and/or other EHR sources.
Coordinates the Disease Management with Primary Care Provider scheduling processes for medical group patients in a centralized department setting.
Uses scheduling systems and understands different protocols for laboratory services at multiple Providence St Joseph Health facilities and freestanding laboratory centers within specified timeframes.
Navigates data system(s) to research and/or record pertinent information in compliance with policies. Reviews patient chart to obtain documentation of requested services.
Receives and appropriately addresses phone calls from patients. Identifies urgent or emergent situations and responds appropriately.
Utilizes motivational and coaching techniques as necessary to remove barriers to care and engage patients in their care.
Uses patient engagement tools such as, phone calls, letters, MyChart messages, and/or IVR solutions.
Works with the healthcare team to identify and continuously improve the quality of services provided and actively collaborate in finding solutions.
Processes all paperwork/forms related to laboratory scheduling.
Contacts specialist offices to obtain clinical information (chart notes, diagnostic results) as needed to assist in disease management process.
Facilitates updating of patient registration information if inaccurate or no longer valid.
Assist providers with appropriate ICD10 diagnosis coding as needed to comply with insurance coverage guidelines of various labs.
Performs other work-related duties and special projects as assigned.
Required qualifications for this position include:
One of the following:
Completion of an accredited Medical Assistant program and registered with State Board (where required by state) or certified through a nationally recognized Medical Assistant program such as AAMA, AMT, RMA, CCMA or NCCT.
Nationally Certified Pharmacy Technician or registered with the State Board.
High school diploma or GED.
Knowledge of medical terminology, disease processes, medical decision making, and prior experience in utilizing disease registries or similar databases.
Excellent verbal communication and interpersonal skills, with preferred experience in motivational interviewing / coaching.
Experience with electronic medical records and/or computerized billing systems.
Epic or other Electronic Health Record /care management software experience.
Computer skills including Microsoft Windows and Office Applications (Word, Access, and Excel).
Excellent typing skills (60 words/minute), computer keyboard, and mouse skills for expedited data processing.
For Medical Assistants - A current valid State Board registration in good standing or Medical Assistant Certification to match education requirements above.
For Pharmacy Technicians - A current valid pharmacy technician license by the State Board of Pharmacy in good standing or National Certification to match education requirements above.
Preferred qualifications for this position include:
For Medical Assistant, a minimum of 1 years of medical assisting work experience.
For Certified Pharmacy Technician, a minimum of 1 years of work experience in primary care or health plan environment.
