Providence Health & Services Social Worker - Hospice in Portland, Oregon
Providence is calling a Social Worker to join our Providence Hospice team to support Yamhill County in Oregon.
Providence Hospice provides experienced, compassionate care for people nearing the end of life. Our doctors, registered nurses, chaplains, social workers and other team members provide everything from pain control and comfort care to emotional, social and spiritual support, including support for family members and caregivers. Our primary goal is to provide the support that people need to allow them to spend their time living as fully and completely as they wish, in their own familiar surroundings, and in the company of family and friends.
While working as a member of the hospice interdisciplinary team, this Social Worker will support Yamhill County service area by providing clinical counseling, assessment, case management, crisis intervention, and psycho-education with multi-stressed patients and families relevant to the dying process, end of life care planning and bereavement.
The ability to understand and provide community-based care across varied residential settings while assessing for patient, caregiver, and staff safety in the home environment is a must.
In this position, you will:
Under direct and continuous supervision, provide comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients, and their support systems.
Incorporate concepts and theories associated with life span development, neurobiology, and behavioral change.
Collaborate with the patient and support system(s) to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter/Multi-Disciplinary Team (IDT).
Perform duties in a manner that respects patient self-determination that reduces health disparities/barriers to care and adheres to the National Association of Social Workers (NASW) code of ethics, Standards of Practice (SOP), and mission.
Required qualifications for this position include:
Master’s degree in Social Work from a Council of Social Work Education (CSWE) accredited school of social work, or from a school in candidacy status by the CSWE
Temporary Clinical Social Work Associates (CSWA) license, or proof of CSWA initiated within 6 months of hire
Documentation of an approved supervision plan from the Oregon Board
Must obtain LCSW Board licensure within a maximum of 5 years from CSWA application, or date of hire date as a CSWA
American Heart Association (AHA) Basic Life Support for Healthcare Provider completion within 1 month of hire
Valid driver’s license and reliable automobile
In compliance with the Providence Health & Services-OR Human Resources policy #302 “Background Checks”, a criminal background check is required for this position, based on assigned duties.
Preferred qualifications for this position include:
Licensed Clinical Social Worker (LCSW)
2 years of clinical work experience in Hospice or Palliative Care
3-5 years of social work experience
2 years of working independently with vulnerable populations in a field environment
