Providence is calling a Senior Manager Elder at Home Operations - Pmg At Home (1.0 FTE, Days) to Providence Office Park Portland in Portland , OR. Anticipated work schedule will be 8:00am to 5:00pm Monday-Friday.
We are seeking a Senior Manager Elder at Home Operations - Pmg At Home that will be responsible for fostering advancement of clinical and non-clinical solutions throughout the Oregon region and the Providence Health Plan (PHP). This includes: providing consultation, public speaking, developing and executing services within the EAH program to advance the risk contract performance. Conducting, applying and disseminating data driven solutions based on national and regional best practice, financial direction, networking to develop internal and external partnerships across the integrated delivery system within Oregon and PSJH and overseeing EAH program managers, staff and team. In this position you will have the following responsibilities:
Direct and oversee the Elder at Home business operations including policy development, staffing, budgeting, and payroll
Hire, train, guide, evaluate and provide leadership and direction for staff to support the day to day operations of scheduled services provided in home and 24/7 availability of appropriate unscheduled services provided in-home
Oversee and ensure effective training and development for Elder at Home Team
Provide regular relevant operational feedback to and from providers, managers, directors and staff involved with Elder at Home delivery so as to influence strategic and tactical modifications and changes in direction
Actively direct and support the effective and comprehensive implementation of the quality strategic plans for Elder at Home delivery and growth, and to improve healthcare outcomes for the Elder at Home cohort across the continuum of geriatric care
Partner with key MD leaders to maintain strong, collaborative relationships across the continuum of care
Direct and oversee the development and implementation of strategies to achieve performance targets including outreach and enrollment and quality care outcomes
Manage organizational performance and Elder at Home service provision in compliance with state, and federal regulatory requirements, and other organizations' requirements
Manage the overall operating and financial success of Elder at Home
Develop goals and annual operating budgets for Elder at Home and ensures Elder at Home delivery is operated within budget guidelines, meets stated objectives and performance targets
Develop and participate in budget/space/capital planning related to operating and capital budgets
Manage and direct the maintenance of information system requirements to support Elder at Home practice and integration/communication with PHP/PMG and others
Monitor and perform regular financial reviews on enrollment, productivity and care outcomes
Develop processes for effectively integrating rising Medicare Advantage members into Elder at Home service delivery in preparation for program growth
Periodically review and analyze the operations of Elder at Home to ensure and improve cost effective operations at all levels
Required qualifications for this position includes:
Master's Degree in Business Administration, Healthcare Administration
Seven (7) years of Progressive health care leadership experience that includes: Healthcare services development, delivery, and operations
Experience leading interdisciplinary care teams, and planning complex geriatric and long term care
Experience developing and executing successful strategic business plans
Experience supervising staff in a matrix structured organization
Experience with Project Management methodologies and software such as CAP, Work-Out and Lean
