Providence is calling a Senior Health Program Director - Population Health (1.0 FTE, Days) to Providence Office Park Portland in Portland, OR
We are seeking a Senior Health Program Director who will who will support program development, data gathering and assessment and community engagement strategies to advance excellence in care for older adults within Providence ministries across Oregon. We are seeking to become an Age Friendly Health System. The Senior Health team and this position will be critical in communicating what that requires and in helping our ministries achieve that goal. This position will engage stakeholders within the community and caregivers who serve patients. In partnership with the Medical Director and other clinicians, this role will develop, assess, and implement current strategies and programs within Providence to better serve and ease access for older adults. The Program Director will make decisions effecting strategy, implementation, and management of resources.
In this position you will have the following responsibilities:
Lead implementation of comprehensive Senior Health strategic plan. Report out to clinical leaders and to regional leadership
Assess markets and determine areas of opportunity / threat within program. Based on analysis, develop recommendations for program and service development / expansion
Stay familiar with clinical and operational developments in the geriatric field through literature reviews, conferences, etc
Provide analysis and perspective on patient data as related to health outcomes and disparities of care
In collaboration with Medical Director, identifies, scopes and leads potential initiatives and approaches to improve health of older adults
Analyze and provides recommendations of strategies to reduce barriers and ease access from marginalized communities of older adults, with special focus on health equity of BIPOC populations.
Develop key performance indicators to determine initiative effectiveness for ongoing monitoring and notify stakeholders of problems affecting timelines and resources
Collaborate with key administrative and physician leaders in each service area and division to gain consensus and to build relationships with existing geriatric programs and expertise
Schedule, coordinate and facilitate appropriate Senior Health council meetings to plan, track and review relevant initiatives and projects. Also coordinate the Patient Family Advisory Council and future Age Friendly Collaborative
Coordinate with PSJH System and Regional teams for application and tracking of internal and external grants.
Engage stakeholders in sensing sessions to engage caregivers in strategy development
Oversight and accountability for Senior Health department budgets including Foundation funds and related grants, to ensure financial performance measures are met
Other duties as assigned
Required qualifications for this position includes:
Bachelor's Degree and Master's Degree in health care, clinical field, public health
Ten (10) or more years senior level management experience in health care, preferably in leading, organizing and managing major clinical programs and business services and functions.
Eight years (8) proven expertise in the ability to lead teams, assert administrative skills, and provide effective direction for excellence in clinical care, while maintaining positive professional relationships
