Providence RN Transfer Center Coordinator in Portland, Oregon
Providence is calling a RN Transfer Center Coordinator (0.0 FTE, Variables) to Providence Oregon Regional Shared Services in Portland, OR.
We are seeking a Registered Nurse (RN) Transfer Center that is responsible for the facilitation and coordination of all requests relating to physician consults, telemedicine requests, and patient transfers between facilities in an efficient, timely, and appropriate manner. Respectfully coordinates provider to provider consults to facilitate appropriate specialist referrals and contacts toward the goal of excellent, timely patient care. Works diligently and collaboratively within a multidisciplinary environment. Provides superior customer service and service recovery, utilizing good judgment and independent thinking that meets or exceeds the expectations of physicians, patients, visitors, and multidisciplinary staff members. Holds responsibility for bed control communications and assignments in an interdisciplinary manner.
In this position you will have the following responsibilities:
Facilitate all hospital-to-hospital transfers/consults as well as assists the [non-clinical] Patient Placement Coordinators with provider-to-provider and telemedicine consults when call volume may prohibit the RN partner from taking all calls in a timely fashion
Coordinate and document communication between referring and receiving physicians, nursing units, and House Supervisors/Throughput Supervisors in a timely manner based upon identified standard
Maintain accurate record of all physician consultation and telemedicine requests
Utilize appropriate clinical judgment and critical thinking skills toward ensuring proper patient placement
Be directly responsible for appropriate bed placement of patients according to medical need and census of nursing unit
Maintain accurate record of all bed assignments on the campuses of PPMC and PSVMC; arranges transfers within EMTALA regulations between acute care facilities; assists with direct admit patient placements between the campuses of PPMC and PSVMC.
Continually monitor and assess bed availability to ensure timely placement; determines whom to accept and/or place in conjunction with House/Throughput Supervisors
Ensure physician has current privileging prior to facilitating transfer or admission
Provide superior customer service and service recovery when customer service standards are not met
Demonstrate the ability and skill to anticipate and diffuse potentially challenging situations and individuals, assuring a positive outcome with critical decision making
Promote Providence St. Joseph Health as a premier service organization by treating customers with compassion and respect
Identify, resolve, and escalate major issues and service failures that impede success
Ensure that compliance is met with federal and state regulatory authorities which include, but are not limited to, The Joint Commission, EMTALA, and HIPAA, in addition to Providence St. Joseph Health policies, procedures, and code of conduct in all aspects of role performance
Required qualifications for this position includes:
Bachelor's degree in Nursing
Current Oregon State Registered Nurse (RN) Licensure
Five (5) years in acute care setting
Preferred qualifications for this position include:
- Leadership experience and/or ED -OR- Critical Care patient care background
