We are seeking a RN Transfer Center Coordinator - Transfer Center who will operate in a multi-functional team that supports the evaluation, consultation, and transfer of patients for any of the eight Providence Oregon hospitals. The Transfer Center Nurse efficiently facilitates complex requests relating to physician consults, telemedicine requests, timely patient transfers between facilities, as well as accurate and efficient patient placement within each hospital and the coordination of transportation. This position collaborates with the Centralized Bed Control team , ministry leadership, incident command and Nursing Supervisors to secure appropriate placement of patients as well as expedite patient flow. This position serves as the central point of contact for regional capacity and is called upon as a leader to disseminate information to area hospitals to manage capacity and patient flow within the region.
Triage patients to the appropriate level of service throughout all hospitals in the Oregon Region
Work diligently and collaboratively within a multidisciplinary environment to innovate and solve for multifaceted issues regarding safe patient care
Provide superior customer service and service recovery, utilizing good sound judgment and independent thinking to meet or exceed the expectations of physicians, patients, visitors, and multidisciplinary staff members in alignment with the caring reliably tones and tools
Coordinate complex transfers of acutely ill patients to Providence facilities from inside and outside our health system
Partner with external transportation companies to ensure appropriate level of care is secured for a safe transport with excellent quality of care
Demonstrate effective leadership and communication skills , both written and verbal
This role is an exempt role that reports to the operations manager of the transfer center.
In this position you will have the following responsibilities
Facilitate all hospital-to-hospital transfers/consults as well as assists the [non-clinical] Patient Placement Coordinators with provider-to-provider and telemedicine consults when call volume may prohibit the RN partner from taking all calls in a timely fashion
Coordinate and document communication between referring and receiving physicians, nursing units, and House Supervisors/Throughput Supervisors in a timely manner based upon identified standards
Maintain accurate record of all physician consultation, transfer, and telemedicine requests
Utilize appropriate clinical judgment and critical thinking skills in assuring proper patient placement
Directly responsible for appropriate bed placement according to medical needs/provider order and capacity of nursing unit
Maintain accurate record of all bed assignments for Centralized Bed Placement; arranges transfers within EMTALA regulations between acute care facilities; coordinates direct admit patient placements
Continually monitors and assesses bed availability to ensure timely placement; determines whom to accept and/or place in conjunction with House Supervisors
Ensure physician has current privileging prior to facilitating transfer or admission
Provide superior customer service and service recovery when customer service standards are not met
Demonstrate the ability and skill to anticipate and diffuse potentially challenging situations and individuals, assuring a positive outcome with critical decision making using the caring reliable tones and tools

Identify, resolve, and escalate safety issues using the chain of command
Ensure that compliance is met with federal and state regulatory authorities which include, but are not limited to, The Joint Commission, EMTALA, and HIPAA, in addition to Providence St. Joseph Health policies, procedures, and code of conduct in all aspects of role performance
Coordinate external transportation of patients and services to support the needs (e.g. Life Flight , Prov Ride etc.)
Utilize information and technology to effectively and efficiently promote patient care, coordinating and communicating with key stakeholder
Effectively collaborates with unit/ministry leadership to problem solve issues related to patient flow and capacity
Provide outstanding customer service/patient experience by answering calls in a friendly and positive manner, keeps hold time to a minimum and caller informed, and resolves all inquiries in a timely fashion
Adapt to changes in a timely and positive manner and lead others
Adhere to all organizational and departmental policies and procedures
Is able to give and receive feedback to improve practice and the environment of care , consistent with the standards of behavior
Teamwork: Work cooperatively and collaboratively with all levels of staff and other disciplines for problem solving without blaming
Focus on the incorporation of safety practices to maintain an atomosphere of continual quality improvement
Required qualifications for this position includes:
Bachelor's Degree in Nursing
Current OR State RN Licensure
Four (4) years in acute care setting
Strong verbal communication and listening skills
Excellent organizational skills
Demonstrate a high commitment to quality
Effective interpersonal skills
Highly thorough and dependable
Effective conflict management skills
Demonstrated project management skills
Preferred qualifications for this position includes:
ED or Critical Care nursing experience
