Providence RN Navigator - Oncology in Portland, Oregon
Providence is calling a RN Navigator - Oncology (0.8 FTE, Days) to Providence Portland Medical Center in Portland, OR.
We are seeking RN Navigator Oncology that will serve as a consistent resource throughout the continuum of cancer care. This RN is knowledgeable on the multiple treatment decisions, strong emotions and financial impact of a cancer diagnosis. The ONN will triage the physical, psychological and social needs of the patient and significant others, offering intervention and /or referrals as needed. Primary goals of this position are to: 1) enable patients with information and education thus reducing a portion of the stress involved in a cancer diagnosis, 2) empower the patient to participate in their personal care process, 3) improve understanding of their decisions and treatment plans; and 4) complete the treatment though the point of survivorship planning. ONN management of care will be provided to ASCT and HD IL-2 patients.
In this position you will:
Work in collaboration with primary referring departments and clinics to assure a smooth transition of patients referred to the program.
Initiate and maintain contact with newly diagnosed patients, through the course of treatment. Provide age appropriate navigation for cancer patients in need of information, care coordination, education (consistent with NCCN treatment guidelines) and resources. Assesses and documents patient and family care needs and, with the patient, formulates and documents a plan of care, or appropriate referrals based on this assessment. Encourage clinical trail participation when appropriate.
Emphasize empowerment of patients through education and support, to advocate for themselves through the treatment phase and beyond to a survivorship plan. Prepare patients for the next steps of their anticipated treatment plan. Coordinate follow-up to various agencies and service providers as needed.
Responsible for initiating oral and written communications to the appropriate team members in a timely manner.
Exhibit skill in prioritizing tasks and assists co-workers with duties, as needed, in a positive manner.
Co-facilitate support groups with social workers as needed. Assist with speaker recruitment. Supply current and appropriate cancer related information to the groups.
Collaborate in development/updating of policies and procedures specific to Nurse Navigation and oncologic disease related programs.
Participate in quality improvement development and implementation; program evaluation and certification; and outcomes research.
Build relationships with referring practitioners to enhance communication, improving the patients and practitioner experience with the Cancer Center. Act as a liaison between health care providers, patients, significant others and payers as necessary.
Actively participate in community events as requested and/or approved by manager.
Provide oncology nursing updates on key cancer issues for Cancer Center social workers and volunteers, participates in nursing service meetings and committees as designated.
Assist with other general issues for the Cancer Center as directed by manager.
Required qualifications for this position include:
Education to meet certification, license or registration requirement
Registered Nurse licensed in the State of Oregon
OCN required within 2 years of hire date
Current BLS certification required
3 years Oncology experience
ASCT experience required of ASCT ONN
Preferred qualifications for this position include:
- Case management or Clinical Trial Nurse experience
