Providence is calling a RN Patient Care Coordinator - Neurological Specialties West (1.0 FTE, Days) to St. Vincent East Pavillion in Portland, OR
We are seeking a Registered Nurse (RN) Patient Care Coordinator - Neurological Specialties West that will coordinate the care of the patient through assessment, planning, implementation and evaluation of appropriate services and resources to maximize optimum outcomes. Work collaboratively with the multiple disciplines, both internal and external to the organization, in effecting quality and cost-effective management of the patient's care along the continuum of chronic neurologic disease.
In addition the position will function as a Clinic Nurse-RN who will be responsible for facilitating the patient care flow in the clinic while providing direct patient care for an assigned provider(s). Will facilitate the workflow in the area by conducting appropriate triage of labs and diagnostic data. The staff member will contribute expertise in the areas of patient scheduling, telephone advice and pharmacy needs. The Clinic Nurse will assist with maintaining compliance to professional nursing standards and regulatory requirements and will support physicians in delivering quality health care to patients taking into account the patients' individual physical or age-related needs.
Manages patient flow by meeting clinic, physician and patient needs.
Continuously prioritizes multiple tasks and completes them in a timely manner.
Identifies medically urgent situations and responds appropriately.
Provides continuity as provider schedules continually change.
In this position you will have the following responsibilities:
Use the nursing process to assess, diagnosis, plan, implement, and evaluate for patients with neurological illnesses.
Collaborate with the referring Neurologist and subspecialty team to facilitate and coordinate services, supplies, and equipment needed by patient and family. This includes involving family, as needed, in team meetings, planning and coordination of care, education, and identification of resources needed to develop realistic expectations of the chronic neurologic care.
Provide appropriate short-term counseling for patients and families whose psychosocial situation demonstrates the need for intervention.
Triage telephone calls and messages, systematically investigating and assessing patient needs determining whether and when patients require appointments, as appropriate, considering age-appropriateness.
Assist patients and families to identify needs for the hospital stay, discharge, and follow-up care.
Required qualifications for this position includes:
Bachelor's Degree in Nursing; from an accredited RN School of Nursing
Current Oregon RN license required
Current CPR certification
3 years healthcare experience -- outpatient, acute care, case management, or long term care, preferred background in home health or case management
