Providence is calling a Health Information Management Specialist - Home Services Specialty Pharmacy (1.0 FTE, Rotating/Variable) to Home Service Building in Portland ,OR.

We are seeking a Health Information Management Specialist - Home Services Specialty Pharmacy that will be responsible for performing tasks that support the health information management functions of Home & Community Care. This includes processing admissions, recertifications, and discharge documentation, receiving and tracking consents, physician orders, and documentation required for certification of eligibility for services, responding to requests for information, scanning and indexing documents into the electronic medical record, and other record retention functions. Work is completed in a manner that supports Health Information Management data integrity standards.

In this position you will have the following responsibilities:

  • Review admissions, discharges, re-certifications, and other clinical documentation for accuracy and completeness

  • Prepare and send physician certifications, orders, and other clinical documentation to the ordering physician for signature

  • Track and follow up on unsigned documentation

  • Receive, review, and file signed certifications, physician orders and other clinical documentation

  • Monitor incoming faxes and routes documents for inclusion in the electronic medical record

  • Scan and index clinical documentation into the electronic medical record

  • Receive, track, and respond to requests for protected health information

  • Receive, scan, track consents, financial disclosures, notices of medicare non-coverage (NOMNC/BIPA) forms

  • Review and performs technical HIM corrections on encounters that are holding claims. When appropriate engages the clinician to correct incomplete documentation

  • Prepare and sends out patient and provider communications (i.e. surveys, discharge letters, medication reports, etc.)

  • Organize, prepare, and destroy paper records following destruction protocols outlined in organizational policies and regulatory rules

  • Maintain document destruction tracking system

Qualifications:

Required qualifications for this position includes:

  • Two (2) years of experience in a team setting which may include participation in a healthcare-related college program

  • Six (6) months of the experience must be in a clerical or general office setting

  • Effective interpersonal skills

  • Assume personal responsibility for actions

  • Able to work within tight timeframes and meet strict deadlines

  • Demonstrate a high commitment to quality

  • Perform work independently with minimal supervision

  • Proactively approaches responsibilities

  • Demonstrated ability to maintain confidential information

Preferred qualifications for this position include:

  • Coursework/Training: Health Information Management, computer science, business, or related health care studies

  • Coursework/Training: Medical Terminology

  • Six (6) months of the experience must be in a clerical or clinical setting

  • Six (6) months of experience using an electronic medical record (EMR)
