Providence Health & Services Patient Placement Coordinator in Portland, Oregon


Providence is calling a Patient Placement Coordinator - Transfer Center (0.8 FTE, Variable/Rotating) to Providence Office Park Portland in Portland, OR.

$1,500 Referral Bonus

We are seeking a Patient Placement Coordinator - Transfer Center who will be responsible of reviewing and assigning bed placements for patients requiring admission to units within an acute care ministry within Providence Oregon. This includes admissions from emergency departments, direct admissions, future admissions, inter-unit transfers, and surgical placements. Chart review and/or calls to clarify bed criteria will be performed to assure appropriate bed placement. The Patient Placement Coordinator collaborates with nursing, physicians, EVS, transportation, and others to define, reduce and eliminate barriers within the system which may impede patient flow. The Patient Placement Coordinator works diligently and collaboratively within a multidisciplinary environment, provides superior communication, customer service and service recovery, utilizes good judgment and independent thinking that meets or exceeds the expectations of physicians, patients, visitors, and multidisciplinary staff members. The Patient Placement Coordinator has responsibility for excellent communication, bed control assignments, and the coordination of transportation scheduling in an interdisciplinary, professional manner.

In this position you will have the following responsibilities:

  • Coordinate and documents inpatient bed placements, communicates with nursing units and House Supervisors in a timely manner based upon identified standards

  • Assist RN Transfer Center colleagues in maintaining accurate record of all physician consultation and telemedicine requests when call volume may prohibit the RN partner from taking all calls in a timely fashion

  • Assist RN Transfer Center colleagues in final patient placement details for transferred patients, including communicating relevant phone numbers for report to the referring facility

  • Directly responsible for appropriate bed placement of patients according to medical need and capacity of nursing unit and hospitals throughout the region

  • Maintain accurate record of all bed assignments across the Oregon region

  • Constantly monitors and assesses bed availability to ensure timely placement; determines whom to accept and/or place in conjunction with House Supervisors, Transfer Center RNs, and RN Expeditor

  • Provide superior customer service and service recovery when customer service standards are not met

  • Demonstrate the ability and skill to anticipate and diffuse potentially challenging situations and individuals, assuring a positive outcome with critical decision making

  • Promote Providence St. Joseph Health as a premier service organization by treating customers with compassion and respect

  • Identify, resolve, and escalate major issues and service failures that impede success

  • Ensure that compliance is met with federal and state regulatory authorities which include, but are not limited to, The Joint Commission, EMTALA, and HIPAA, in addition to Providence St. Joseph Health policies, procedures, and code of conduct in all aspects of role performance

  • Use effective communication and interpersonal skills to collaborate with key persons and departments to facilitate and maximize patient flow

  • Be responsible for facilitating forward patient flow for all patients

  • Able to act independently and apply knowledge of hospital, unit admission criteria, and type of care provided in each department to appropriately place patients by utilizing critical thinking and problem solving skills

  • Attend ministry and regional huddles to understand inter-unit patient placement needs, staffing, capacity and to communicate any patient placement needs or issues

  • Establish good rapport and cooperative working relationships with personnel in the organization and the community for which Providence serves

  • Maintain positive, collaborative interactions with physicians, department leaders, and staff members to determine the needs of the facility and community

  • Provide outstanding customer service/patient experience by answering calls in a friendly and positive manner, keeps hold time to a minimum and caller informed, and resolves all inquiries in a timely fashion

  • Provide accurate and timely follow up with staff, providers, patients, families, and sending facilities

  • Adhere to all organizational and departmental policies and procedures

  • Adapt to change with a willing and innovative spirit

  • Provide clear, concise, pertinent reports/information

  • Communicate effectively with patients, physicians and staff in a professional manner


Required qualifications for this position includes:

  • Associate's Degree -OR- equivalent education/experience

  • One (1) year Health care experience

  • Demonstrated problem solving skills

Preferred qualifications for this position includes:

  • Bachelor's Degree in a healthcare field

  • Bed placement experience

  • Registrar experience

  • Possess a strong technical aptitude
