Patient Navigator - Providence Womens Clinic East Portland (1.0 FTE, Day) to 545 Building in NE Portland, OR.

Schedule Details: 8:00am - 5:00pm, Monday-Friday, No weekends

We are seeking a Patient Navigator - Prov Womens Clinic E Portland who will coordinate services and guide pregnant women across the continuum of care to help achieve the best possible quality, experience and affordability outcomes for mothers and newborns. The Patient Navigator work as part of an integrated care team that includes midwife and OB providers, clinic staff, social workers, financial counselors and educators to effectively address the needs of women throughout their pregnancy and birth experience in a collaborative, patient-centered environment. The Patient Navigator has a solid understanding of the various Providence-provided and community-provided resources that relate specifically to pregnancy care and can advise patients on how to best take advantage of these resources.


Required qualifications for this position includes:

  • Three (3) years work Experience in a medical clinic, office setting or administrative setting

  • Five (5) years work experience as Medical Office SSC or Coordinator

Preferred qualifications for this position includes:

  • Bachelor's Degree

  • Education in a health-related field, e.g., public health, sociology, psychology or health education

  • Two (2) years Experience in the appropriate specialty setting

  • Experience facilitating group sessions and/or discussions
