Providence Health & Services Nurse Practitioner Palliative Care in Portland, Oregon
As Nurse Practitioner with Connections Palliative Care, you’ll join an established interdisciplinary team providing a single, coordinated program for management of chronic conditions or life-limiting illness to patients and families. Seeking an Oregon-licensed, board-certified Nurse Practitioner with a minimum of 1-year of post-graduate clinical experience in palliative care, hospice, geriatrics, and/or oncology preferred. Connections Palliative Care is part of a growing integrated Home and Community Services program which collaborates with Hospice, Home Health, Geriatrics as well as hospital-based practitioners to care for patients and their families.
Part-time at 4 days per week
Outpatient care setting, community-based settings, and home visits and occasional hospital setting
Collaborate with physicians, nurse care manages, social workers, pharmacists, behaviorists, and other team members
1-years of post-grad clinical experience in palliative care, hospice, geriatrics, and/or oncology preferred

Providence Connections is a palliative care consultation service. Our interdisciplinary teams help patients and their families find needed resources to control symptoms associated with their illness and maximize quality of life while providing compassionate support. Our teams of physicians, nurse practitioners, palliative care nurses, clinical social workers and chaplains work collaboratively with all members of patients’ care teams to provide whole person care.
