Providence is calling a LPN -Gresham Nursing Services (1.0 FTE, Days) to Providence Elderplace Oregon in Portland, OR
We are seeking an Licensed Practical Nurse (LPN) - Gresham Nursing Services who will provide quality nursing care in a clinic or community setting, under the direction of a Registered Nurse (RN). Perform all duties within the standards and scope of practice for the Licensed Practical Nurse (LPN). Function within the Interdisciplinary Team (IDT) to formulate and evaluate the Participant’s Plan of Care (POC). Complete focused assessments, by collecting both subjective and objective data. Assess educational needs and develop trainings for participants, families and caregivers. Identify areas of need or potential change in a participant’s status. Perform clinical tests, treatments, therapies, medication administration, health teaching and counseling, as directed by the Primary Care Physician (PCP) and RN.
In this position you will have the following responsibilities:
Provide direct participant care, including but not limited to: focused assessments, medication administration/management, clinical treatments, venipuncture, point of care lab testing, catheter care, foot care, parental infusion therapy and provider support
Administer care based on the nursing process, the interdisciplinary and nursing care plan
Assist the RN with care management functions, including tracking, risk mitigation techniques, and care plan monitoring and follow-ups
Contribute to the care plan by making pertinent nursing observations and participating in the multidisciplinary team care conferences, staff meetings, and in-services as indicated
Identify and follow the appropriate standard of care for the participant's medical or nursing diagnosis
Be accurately and concurrently documents care given
Documentation comply with nursing standards, the interdisciplinary care plan and approved EPIC workflows
Maintain a current working knowledge of PACE rules and regulations
Required qualifications for this position includes:
Current unencumbered Licensed Practical Nurse (LPN) license by the Oregon State Board of Nursing
American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider certification required within one (1) months of hire
Preferred qualifications for this position include:
- Experience in a clinic or clinic-like setting
