Providence Health & Services Home Service Liaison in Portland, Oregon
Providence is calling a Home Services Liaison (1.0 FTE, Days) to Providence Home Services Building in Portland, OR.
In this position, you will:
Coordinate home medical equipment (HME) services for patients being discharged from a designated hospital.
Work at an assigned hospital with hospital-based Acute Care Managers (ACM), Patient Care Coordinators (PCC), discharge planners, and Medical Social Workers (MSW).
Collect and prepare all necessary documentation required for a complete order; communicates with Access Services to complete the coordination of the order.
Act as a resource and educates hospital staff regarding the services offered by HME and contributes to the patient care plan as it relates to the discharge plan.
Provide information to ensure a smooth transition for patients and their families following hospitalization.
Required qualifications for this position include:
High school diploma or equivalent
2 years experience in the home medical equipment (HME) field
2 years experience in HME customer service or HME insurance verification
In compliance with the Providence Health & Services-OR Human Resources policy #302 “Background Checks”, a criminal background check is required for this position, based on assigned duties
Preferred qualifications for this position include:
- Familiar with the setup and operation of standard HME items like walkers, canes, crutches, commodes, basic wheelchairs
