Providence is calling a Clinical Pharmacist Oncology Specialist - Psv Oncology Clinic (1.0 FTE, Rotating/Variable) to Saint Vincent Mob East Pavillion in Portland , OR. This position's shift will be M-F 4 10-hour shifts (day off varies) that start as early as 7am and end as late as 6pm.

We are seeking a Clinical Pharmacist Oncology Specialist - Psv Oncology Clinic that will be responsible for coordination of medication-related services across Providence Oncology and Hematology Care Clinics. Responsibilities include promotion of evidence-based, safe, effective drug therapy, standardization of medication practices across clinics, and coordination with drug procurement, inventory management and billing processes.
In this position you will have the following responsibilities:
Provide comprehensive pharmaceutical care to hematology/oncology patients
Discuss medication order clarifications with prescriber, document any changes in patient medical and pharmacy records, and inform others of medication order changes as necessary
Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions
Provide pharmacokinetic consultations as required
Prevent, detect, monitor, document and reports adverse drug reactions and medication errors
Required qualifications for this position includes:
Graduate from an accredited College of Pharmacy
PharmD - OR- Master's Degree Pharmacy; with residency -OR- equivalent education/experience
Current valid pharmacy license by the Oregon State Board of Pharmacy
Ability to become eligible for the Board Certified Oncology Pharmacist exam within two (2) years of hire
One (1) year of Hematology/Oncology experience
Preferred qualifications for this position include:
PGY-2 in Hematology/Oncology
Board Certified Oncology Pharmacist
