Providence Health & Services Clinical Manager - Hospice Social Work in Portland, Oregon

We are seeking a Clinical Manager Hospice - Hospice Access that will serve as the manager of a clinical program and provides staff supervision and coordination for the care provided to patients and their families served by a designated hospice interdisciplinary team. This position is responsible for identifying 'best practices' for care. As well, this position may network with community resources and consult with community health care providers serving patients with life threatening illness.

In this position you will have the following responsibilities:

  • Provide clinical oversight to interdisciplinary team members for designated team with the support of a discipline supervisor as required for discipline-specific input

  • Triage patient’s needs and leads staff in clinical problem-solving as needed; assigns clinicians to patients; oversees daily assignment of clinicians visits that need to be covered when primary clinicians is out of office or unavailable

  • Identify staffing and staff development needs; works with ministry Director, Sr Manager, and Hospice Operations for staffing approval or to otherwise address and resolve issues

  • Hire, schedule and manage performance of the assigned team, including nurses, social workers, chaplains, home health aides, and other clinical disciplines

  • Facilitate team Interdisciplinary Group Meeting (IDG) and team meetings

  • Integrate input of IDG when making clinical or administrative decisions such as coverage for requested treatments and therapies, changes in level of care, patient discharge, etc

  • Facilitate annual team or discipline strategic planning retreat

  • Ensure regulatory compliance for assigned Care team; participates in development of policies and procedures to ensure compliance in accordance with standard PHHC procedures, Medicare Conditions of Participation and other regulatory agencies

  • Be responsible for understanding patients’ insurance benefits and managing care in a cost effective manner; and approving expenses for care

  • Identify trends within team’s provision of care as well as recurring issues for particular disciplines; integrates and introduces new processes and procedures in partnership with appropriate staff

  • Participate in community activities to provide information regarding clinical practice area to increase the understanding of hospice care to physicians, other health care providers and to community

  • Participate as a member of various management and leadership team meetings and shares rotating responsibility for “Administrator On-Call” support

  • Possibly network with community agencies to build relationships with referral and care sources and coordinate optimum care for patients and their families

  • Possibly assist in the development of resource materials regarding care of terminally ill patients; develop and present in-services and training opportunities to staff, clients, community health care providers, and the public

  • Possibly participate in strategic planning for the ministry; establish team priorities, plans and annual budget; develop and revise policies and procedures for the team in keeping with standard PHOS procedures


Required qualifications for this position includes:

  • Bachelor's Degree in Health care related field -OR- Master's Degree in Social work or Spiritual Care

  • Current licensure in the applicable discipline (e.g., Nursing, Physical Therapy) required in the state of Oregon. Staff who provide service in the state of Washington require Washington state licensure in the applicable discipline.

  • Current driver's license and automobile insurance.

  • Current Healthcare Provider CPR certification

  • Three (3) years of Clinical experience in chosen field - Social Work

Preferred qualifications for this position includes:

  • Hospice certification or advanced certification

  • Hospice experience

  • Master's Degree in Social Work

  • Three (3) years of staff supervision experience

  • Proficient in using computers and Microsoft Office Suite programs required; knowledge of EMR
