Providence Patient Navigator - OBGYN in Polson, Montana


The Patient Navigator serves as clinical expert coordinating the patient care, community resources and program outreach for perinatal mental health and substance use. The Patient Navigator will assist patients and their families in managing through the services and programs offered by the hospital, clinic, and the community to meet their health and wellness needs. The Patient Navigator will tailor services to the unique wellness needs of the patients served by connecting to resources to address Social Determinates of Health, assessing for barriers to care, mental health screening tracking and trends, and lactation services. The Patient Navigator will participate in health screenings, provide patient education, and offer appropriate referrals. The Patient Navigator will be responsible for cultivating relationships with patients and their families, physician partners, behavioral health, members of the healthcare team, and community partners. The patient navigator will organize and coordinate team care coordination meetings, update care team on challenges and clinical benchmarks not being met and be responsible for collecting data related to contacts and referrals as outlined by department manager. The Patient Navigator will consistently demonstrate behaviors that model the SJMC core values and the hospital's mission statement.

Required qualifications:

  • Certified Lactation Consultant within 1 year of hire

  • 1 year worked within a health-related field

  • Some examples include but are not limited to Medical Assistant, Emergency Medical Technician, Certified Nursing Assistant, Pharmacy Tech or Social Work. Work experience demonstrates ability to work independently/autonomously within a team

Preferred qualifications:

  • Associate's or Bachelor's Degree or

  • Bachelor's Degree in Psychology, sociology, nursing, community health

  • 6 months preference will be given to applicants who have experience working with hard to reach populations or with identified 'high need' communities
