Providence Health & Services Licensed Addiction Counselor in Polson, Montana
Providence is calling a Licensed Addiction Counselor (Behavioral Health, Days, 1.0 FTE) to Providence St. Joseph Medical Center in Polson, Montana. In this position you will:
Assess patient/family learning needs, abilities, preferences and readiness to learn. Completes on going assessments and documents according to policy. Communicates with other members of the health care team regarding the patient’s needs, plan and response to care. Evaluates response to plan and makes adjustments as indicated.
Assess, collaborate and coordinate patient’s discharge planning needs and readiness for discharge with other members of the care team.
Consult regarding cases that are presenting problems and/or experiencing significant deviations from the plan of care, conferring with the Medical Director/Department Director as needed.
For the ages listed, screens, assesses, monitors, documents and interprets psychosocial, spiritual, environmental, self-care, educational, and discharge planning needs.
In collaboration with the patient/family, and in a developmentally and culturally appropriate manner, implements the plan of care according to established Social Work policy.
Maintain current knowledge of reimbursement and regulatory issues of the payors most frequently seen with the patient population.
Obtain a history through the interview process, maintains ongoing monitoring/assessment of assigned patients.
Participate in the development of patient plan of care to include patient problems, goals, evaluations and interventions.
Report symptoms and significant changes in patient’s condition to appropriate personnel.
Utilize appropriate teaching methods and/or referrals to ensure patient/family educational needs are met. Provides community resource information to patient/family and evaluates the effectiveness of teaching, making adjustments as necessary. Documents education process on appropriate screen/format.
Work with health care team and outside resources to facilitate optimum patient care delivery.
Educate health team colleagues about Social Work. Including the role and unique needs of the case-managed patient populations and serves as a resource for physicians and other departments.
Required qualifications for this position include:
Master's Degree or Bachelor's Degree in Professional Counseling.
LCPC (Licensed Clinical Professional Counselor)
Three (3) years experience in similar work structure and level of clinical responsibility.
Well developed therapeutic skills, particularly in the area of group therapy. Must perform effectively under stressful patient/counselor therapeutic relationships. Must have writing skills capable of maintaining extensive documentation of case medical files.
Preferred qualifications for this position include:
Eligible for licensure in the state of Montana preferred, eligible for LAC (Licensed Addiction Counselor) or LCSW.
Addiction Counseling or Substance abuse experience.
