Community Health Worker I
The Community Health Worker I (CHW-I) is responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors.
Qualifications:
Required qualifications:
Valid driver's license
For CHWs providing outreach and enrollment, become a Certified Enrollment Counselor (CEC) and be certified to enroll into Cal Fresh within 3 months of hire.
1 year of working experience
