Providence Health & Services Patient Safety Specialist - Quality Assurance in Olympia, Washington
Providence is calling an Patient Safety Specialist (0.8 FTE) to Providence St. Peter Hospital in Olympia, WA.
The Patient Safety Specialist (PSS) assists with developing and administering interventions that support a safe environment for patients of Providence Health & Services in the Southwest Washington Service Area (SWSA). Through proactive risk assessment, event investigation, data analysis, and continuous quality improvement activities, the PSS works with SWSA leaders to identify risk areas and mitigation strategies. The PSS provides consultation and education to leaders and members of the care delivery team to help create a safe and reliable environment of continuous quality and process improvement.
In this position you will:
Lead serious safety event investigations and improvement activities in conjunction with managers: provide support and just in time training to operational mangers for precursor and near miss safety events
Promote and model a strong patient safety culture via education on routine and as needed basis:
-teach HRO Toolbox for Everyone and HRO Leader Methods to new leaders and providers.
-teach High Reliability intensives to update and refresh all caregivers on Tools and Tones and as needed.
-round on staff and providers and teach error prevention tools when appropriate
Encourage event reporting across all levels of the organization so that processes can be evaluated and patient safety improvements identified.
Facilitate small group sessions to promote High Reliability leader tools and accountability.
Conduct assessments with a non-punitive approach to mistakes and accidents that fosters open communication among all disciplines
Conduct process mapping for events and create a collaborative culture around event investigations
Participate in, and lead when necessary, various types of event analyses to include (but not limit to): failure mode and effects (FMEA), root cause analysis (RCA), intensive analysis, serious safety event reviews and apparent cause analysis (ACA).
Assist with Culture of Safety survey
Coordinate with data analytics to assure appropriate metrics are assigned for continuous quality improvement activities
Analyze event trends and identify areas of patient or caregiver risk that require mitigation strategies
Required qualifications for this position include:
Required: Bachelor’s degree in a clinical discipline or healthcare-related field
Minimum five (5) years’ healthcare experience in in an acute care or ambulatory care setting; including a minimum two years with direct patient care.
Preferred qualifications for this position include:
Previous quality experience with PDSA, previous experience with patient safety
Certified Professional in Patient Safety (CPPS), Certified Professional in Healthcare Quality (CPHQ), Lean Six Sigma certification
