Providence Health & Services Outreach Specialist - Emergency Services in Newberg, Oregon

Providence is calling an Outreach Specialist to support the PMG Family Medicine Group patients that have high utilization of Emergency Services at the Providence Newberg Medical Center's Emergency Department in Newberg, OR.

Providence understands that people with high utilization of emergency services tend to lack resources that impact their health outcomes, such as food, housing, or social supports and services. Providence is hiring an Outreach Specialist to help provide additional care coordination, resources and support.

The Outreach Specialists will have a primary office space at the PMG Family Medicine Clinic within Providence Newberg Medical Center. This position will require the use of a personal vehicle to travel to and meet program participants in the community. The schedule for this position will be during the day hours Monday - Friday. This position will be for 40 hours a week.

In this position you will:

  • Work with a vulnerable population that has a high utilization of the Emergency Department.

  • Work in conjunction with patients, team members, hospital staff and community partners to develop and implement strategies that improve the health of individuals who are identified with high utilization of emergency department services.

  • Organize, collect, review and report health and social information through member home visits and phone outreach, while demonstrating multicultural sensitivity and effective communication skills.

Qualifications:

Required qualifications for this position include:

  • Bachelor's degree

  • 2 years of experience working with diverse vulnerable populations, community and/or health care organization experience

  • American Heart Association (AHA) Basic Life Support for Healthcare Provider completion, or obtained within 1 month of hire

  • Valid driver's license and reliable transportation

  • Knowledge of State social service agencies and community resources

  • Knowledge of health systems, health education, motivational strategies, and an empathetic manner working with the underserved

  • In compliance with company policy and state regulations, a Department of Human Services (DHS) criminal background check may be required for this position based upon the location of the position. Fingerprinting may be required also.

Preferred qualifications for this position include:

  • Degree in social work, social sciences or human services fiel

  • Direct care experience in a residential and/or outreach position serving vulnerable populations
