Providence Health & Services RN Central Intake Coordinator in Mount Angel, Oregon

Providence is calling a RN Central Intake Coordinator - Home Health Services (1.0 FTE, Days 40 hours a week) to Benedictine Nursing Center in Mid Willamette Valley area .

This opportunity has a Sign on bonus of $5,000, that will be paid out in 2 installments over the spand of 12 months from your start date .

We are seeking a RN Central Intake Coordinator - Home Health Services who will be the main access point for all Providence Benedictine Home Services customers to order comprehensive home care services on a local and regional basis. The Central Intake RN arranges prescribed services with the physician, payor, patient/caregiver, and provider to ensure that services are provided in a timely and coordinated fashion. Comprehensive services include home health, home infusion (pharmacy and nursing), and enteral therapy. The Central Intake RN works closely with the Authorization Specialist to obtain the preauthorization for home health services for subcontract agencies for some of Providence Home Services regional contracts

In this position you will have the following responsibilities:

  • Identify by telephone, patients requiring home health, home infusion pharmacy and nursing, and enteral therapy. If a patient is deemed to require other services not offered by Providence Home Services, the Central Intake RN gives the referral source information on appropriate community resources.

  • Take referral information either by phone or fax and may access other necessary information on-line (e.g. history and physicals, inpatient notes/summaries, and labs, and physician names and addresses) to complete the referral

  • Work closely with the Insurance Verification and Authorization Specialists to investigate and document insurance eligibility and benefits and required authorizations for all referrals

  • Communicate information to patients/caregivers, physicians, and providers and assists in making alternative arrangements as dictated by insurance coverage and benefits. At time of referral, obtain complete insurance information including name of insurance, name of insured person and relationship to patient, group and ID number of insurance, place of employment for insured member, and contact information for verification of insurance eligibility and benefits

  • Work with clinical liaisons, hospital discharge planners, and extended care facility care managers , physician offices, and other referral sources to plan and organize home services for patient discharges in the local and regional service area

  • Arrange and coordinate services provided directly by Providence Benedictine Home Services and by subcontractors, or by other providers as dictated by patient choice or insurance requirements

  • Complete all required information into the Home Services Medical Record system for the clinical providers to ensure that the ordered services are rendered timely

  • Manage insurance-specific home health cases referred to subcontract providers

  • Communicate with the subcontract providers and payors/case managers as required to ensure that services rendered are skilled and medically necessary

  • Maintain accurate daily referral log for North and South geographic teams and compiles diverted log with reasons for Home Health Director

  • Establish and maintain positive working relationships with home care providers, payors/case managers, physicians and their office staff, hospital and extended care facility care managers and discharge planners, and Providence Home Services departments

  • Focus on being customer service oriented at all times

Qualifications:

Required qualifications for this position includes:

  • Current, unencumbered Oregon RN license

  • Valid drivers license

  • Driving record consistent with Safe Driving Policy 330 Incident Point Matrix at time of hire

  • An established record as a strong team player and the ability to work closely with others

  • Knowledge of comprehensive home care services and supporting community organizations

  • Strong organizational, analytical, and communication skills

  • Able to work independently, to coordinate multiple facets of patient care, to prioritize work, and to manage time to accomplish multiple tasks in an active office atmosphere with multiple interruptions

  • Proficiency in the use of laptop computer and multiple business software applications

Preferred qualifications for this position includes:

  • Bachelor's Degree in Nursing or Social Work.

  • Required for newly hired RNs as of 2018. Degree in related field may be considered

  • General knowledge of state and federal regulations and JCAHO and safety standards for home care services

  • An understanding of how the health care delivery system works and the interrelationships among patients, physicians, hospitals, home care providers, community agencies, suppliers, and third party payors
