Providence Health & Services Ambulatory Pharmacist - Credena Health Pharmacy Monroe in Monroe, Washington


Credena Health Pharmacy Monroe is calling an Ambulatory Pharmacist (1.0 FTE, Variable Shifts) in Monroe, WA.

Searching for a clinical-minded pharmacist to work in the Credena Health Pharmacy Monroe outpatient pharmacy. Perform professional pharmaceutical dispensing functions by providing prescribed medications, drugs, and other pharmaceuticals according to standards and practices for patients in all age groups including neonatal, pediatric, adolescent, adult and geriatric. Promote safe, cost-effective, rational drug use and provide expert pharmaceutical consultation to other healthcare colleagues. Promote new clinical services to provide innovative care to our patients.

Demonstrates age specific patient care considerations for the population served:

• Demonstrates application of knowledge, skills, and behaviors in the physical, motor/sensory, cognitive, psychosocial, patient and family education areas based on PRMCE age specific care competency assessment tool.

Participates in the development, implementation and monitoring of a pharmaceutical care plan to reach a desired outcome for each patient:

• Directs the control of medication administration and drug distribution for outpatient services for patients in all age groups including neonatal, pediatric, adolescent, adult and geriatric.

• Reviews orders prior to verification for any possible interactions, duplications or other contraindications.

• Ensure all medication are dispensed promptly and accurately.

• Reviews issues with patients as appropriate.

Promotes cost-effective guidelines and individualized drug therapy:

• Documents therapeutic interventions in the electronic medical record.

• Manage requests for non-formulary medications in an appropriate and cost-effective manner.

• Decisions for implementing drug use are based on evidence-based principles.

Follows written protocols within the Washington State Department of Health regulations to determine appropriate individual therapy:

• Chart audits indicate appropriate decisions.

Minimizes or prevents the incidences and severity of adverse drug effects:

• Documents therapeutic interventions in the electronic medical record.

• Identify and appropriately act on potential adverse drug effects.

Provides oversight and direction to staff:

• Directs and oversees all aspects of the medical distributing process system.

• Professionally precepts and/or trains new staff, pharmacy residents and students.

• Appropriately directs and accurately checks the work of Technicians and Assistants.

Provides drug information and education (written or oral) to patients and other customers:

• Documents therapeutic interventions in the electronic medical record. Chart audits indicate appropriate use of drug information resources.


Required qualifications for this position include:

  • Bachelor of Pharmacy degree from an accredited school of Pharmacy

  • Licensed as a Registered Pharmacist in the State of Washington

  • BLS certification must be obtained within one (1) year of hire

Preferred qualifications for this position include:

  • Doctorate of Pharmacy degree from an accredited school of Pharmacy

  • Completed American Society of Health-Systems Pharmacists (ASHP) accredited postgraduate year one (PGY1) Residency Program or three (3) years of acute care work experience
