Providence Health & Services Discharge Planner in Missoula, Montana
Providence is calling a Discharge Planner (Mental Health Adolescent, Day Shift, 1.0 FTE) to Providence St. Patrick Hospital in Missoula, Montana. In this position you will:
Assess patient/family learning needs, abilities, preferences and readiness to learn. Complete on going assessments and documents according to policy. Communicate with other members of the health care team regarding the patient’s needs, plan and response to care. Evaluate response to plan and makes adjustments as indicated.
In collaboration with the patient/family, and in a developmentally and culturally appropriate manner, implements the plan of care according to established Social Work policy.
Obtain a history through the interview process, maintain ongoing monitoring/assessment of assigned patients.
Participate in the development of patient plan of care to include patient problems, goals, evaluations and interventions including after-care planning.
Report symptoms and significant changes in patient’s condition to appropriate personnel.
Utilize appropriate teaching methods and/or referrals to ensure patient/family educational needs are met. Provide community resource information to patient/family and evaluates the effectiveness of teaching, making adjustments as necessary. Document education process on appropriate screen/format.
Work with health care team and outside resources to facilitate optimum patient care delivery.
Required qualifications for this position include:
- Bachelor’s degree from an accredited school of Social Work or related field
Preferred qualifications for this position include:
- 3 years social Work experience. Strong professional level of knowledge and comprehensive clinical assessment skills
