We are seeking a Care Management Associate who will collaborate with the patient/family, the acute multidisciplinary team, and post-acute providers to implement the patients discharge plan. The CMA also proactively escalates risks and barriers to discharge to the Case Manager; documents all discharge planning efforts in the patient's medical record in a timely manner; maintains ongoing communication with the multidisciplinary team and follows all regulatory requirements and ministry polices regarding care coordination/discharge planning. .
In this position you will have the following responsibilities:
Adheres to EMTALA, Title 22 and other Regulatory Requirements related to patient care.
Coordinates referrals based upon patient's discharge goals, discharge needs, patients payor benefits and payor/government regulations.
Coordinates post-acute services including SNF, Residential Facilities, Home Health, DME, Transportation, Follow-up appointments, etc.
Promotes the use of alternate care settings in order to provide the Hospital with optimal reimbursement while providing quality and continuity of care for our patients.
Assists with high-level and lateral transfers.
Provides timely updates to patient/family and multidisciplinary team regarding status of implementing the discharge plan.
Responds to calls for information from payers, and follows up on existing payer request. Contacts appropriate care manager as required.
Assures appropriate documentation is completed on patient records as identified in department policies.
Be a resource to patients, families, and the multidisciplinary team regarding post-acute services.
Demonstrates willingness & ability to float to areas within their scope of practice as necessary.
Demonstrates responsibility and accountability for own professional practice in regard to:
Attendance, punctuality and patient confidentiality.
Positive public relation image and working relationships.
Obtaining necessary knowledge and skill needed to maintain competence and certifications.
Completing all mandatory requirements, including annual safety programs and annual physical.
Participates in the development and attainment of team, department and hospital goals including:
Attending and participating in department meetings.
Volunteering or accepting assignments for projects.

Required qualifications for this position include:
Completed an LVN program from an accredited school -OR- 10 years or more care management experience -OR- completed Bachelors degree
2 years experience in health care setting (acute or post-acute)
2 years LVN experience (if LVN)
2 years experience in health care case management discharge planning
Strong verbal communication and listening skills
Demonstrated customer service skills
Highly thorough and dependable
Seeks to acquire knowledge in area of specialty
Excellent organizational skills
Demonstrated time management and priority setting skills
Demonstrated ability to maintain confidential information
