We are seeking a Registered Nurse Palliative Care that will provide services to inpatient populations. Under the supervision of the Medical Director, the Registered Nurse Palliative Care provides services to palliative care patients and their families following nursing processes and in accordance with the Nurse Practice Act. The Nurse participates as a member of the interdisciplinary team. In the hospital setting, the Nurse participates in the identification of appropriate patients and serves as a liaison between the Palliative Care team and other staff caring for hospitalized patients. The Nurse facilitates family conferences to assure seamless transitions for patient and family across changes in care settings, care providers and care goals. In the home and long-term care settings, the Nurse provides support to patients with life-limiting illness and their families. The RN will participate in the evaluation of Palliative Care patients for transition to hospice programs.

In this position you will have the following responsibilities:

  • Maintain the standards of Nursing in accordance with the principles set forth by the Oregon State Health Department, Oregon State Board of Nursing, The Joint Commission, NHPCO, and agency policies and procedures

  • Serve as a liaison between the Palliative Care team and hospital staff

  • Utilize nursing process, assess patient status and works with attending staff to intervene appropriately, and evaluates patient response

  • Participate as a member of the interdisciplinary team addressing patient care and family issues

  • Observe, evaluate, and bring to team conferences information regarding psychosocial, spiritual, physical and financial conditions affecting the patient and family

  • Serve in the role of advocate for the patient/family unit

  • Develop and maintains contact with appropriate community agencies and services in order to promote inter-agency cooperation and to facilitate related referrals

  • Facilitate and support decision making around end of life planning, including discussions about ending treatment, advanced directives and communicating wishes with family and friends

  • Maintain patient records in accordance with Palliative Care, State and Federal requirements

  • Engage in professional development related to position responsibilities


Required qualifications for this position includes:

  • Valid unencumbered Oregon Nursing License

  • Current American Heart Association Basic Life Support for Health Care Provider certification

  • Valid driver's license

  • Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire

  • Experience in addressing end of life medical, social and legal issues

  • Experience referring to community services

  • Three (3) years of clinical experience in a hospital, hospice, or palliative care setting

Preferred qualifications for this position include:

  • Bachelor's Degree in Nursing
