Providence Health & Services Patient Guide - Emergency Services in Medford, Oregon


Providence is calling a Patient Guide to the Emergency Department at Providence Medford Medical Center (PMMC) in Medford, OR.

As a member of the Care Management Team, the Patient Guide works in the emergency department. Patient Guides use a list of criteria and the Electronic Medical Records system to identify patients that need assistance with obtaining insurance, identifying a primary care or are frequently accessing services through the emergency department for treatment that could be accessed through primary or urgent care. Once identifying those with these needs, the patient guide interacts with the patients to offer resources and information about community based services and assist with establishing insurance providers.

This is a full time, non-exempt position that will be scheduled from Monday through Friday, 12pm - 8:30pm PDT.

In this position, you will:

  • Coordinate alternative services for patients seeking emergent care within the Emergency Department in situations where other avenues of care are more appropriate to affect optimal patient outcomes, achieve continuity and quality of care and reduce costs.

  • Facilitate patient/family education which will be a key component of success.


Required qualifications for this position include:

  • Associate's degree or equivalent experience

  • 3-5 years of customer service experience, with at least 1 year of the experience in healthcare

Preferred qualifications for this position include:

  • Experience with patient contact
