Providence Health & Services Weekend Physical Therapist - Home Health Sound in Lacey, Washington
Providence is calling a Weekend Physical Therapist (PT) (0.5 FTE, Day Shift) to Home Health Sound at SCS Home Health in Lacey, Washington.
The Physical Therapist is directly responsible for planning and administering medically prescribed physical therapy treatment programs in the home settings for patients to restore function, relieve pain and prevent disability following disease, injury or loss of body parts. The physical therapist consults with the physician in developing a plan of treatment including specific procedures and equipment to be used and frequency of visits.
In this position you will:
Describe assessment & accurately interprets data holistically (psychological, physiological, cognitive & medication regime)
Determine priorities of client care based on client's needs and available resources: Manages time effectively. Prioritizes client's needs effectively.
PT care needs are assessed as needed for PT clients: After a referral, an accurate history and physical exam are performed. Documents assessment data within 24 hours of home visit.
Written plan of care developed which best meets the client's needs holistically
Define the coordination of care needed
Cooperatively assists clients & significant others in developing a realistic plan of care
Provides treatment interventions safely and competently: Delivers care according to care plan & established priorities. Practices consistent with SHC approved procedures, protocols & standards of care. Plans and completes home visit in timely manner. Notifies supervisor when deviations occur outside the norm. Meets time management and productivity standards.
Required qualifications for this position include:
Degree in Physical Therapy
One (1) year experience in home health
Current Washington State Physical Therapy License
Valid Washington State driver’s license
Insured and reliable transportation
Healthcare Provider BLS/CPR certification
Preferred qualifications for this position include:
Two (2) years of therapy experience with adult physical disabilities
One (1) year experience with the geriatric population
