Providence is calling a Social Worker (Hospice Sound, Day Shift, 1.0 FTE) to Providence Sound Home Care and Hospice in Lacey, Washington. In this position you will:
Psych/social care needs are assessed as needed for SLP clients: After a referral, an accurate history and physical exam are performed. Documents assessment data within 24 hours of home visit.
Describes assessment & accurately interprets data holistically (psychological, physiological, cognitive & medication regime).
Observes reimbursement eligibility criteria in implementing services for clients: Assists in obtaining information to determine alternative payment sources. Communicates with insurance company or HMO's to determine eligibility for home care.
Determines priorities of client care based on client's needs and available resources: Manages time effectively. Prioritizes client's needs effectively.
Written plan of care developed which best meets the client's needs holistically.
Required qualifications for this position include:
Master’s degree MSW from a school of Social Work
One (1) year experience in a health care setting
Three (3) years Post Master's work experience, or prior experience in hospice care setting
Current LASW or LICSW Social Work License in Washington State
Current Health Care Provider CPR certification
Current Washington State Driver's license
Valid automobile insurance
Preferred qualifications for this position include:
Two (2) years experience in a health care setting
Prior experience in home health/hospice care setting
