Providence LPN - Pace Alder in Kent, Washington
Providence is calling a Licensed Practical Nurse (LPN) to support PACE (Program for All Inclusive Care for the Elderly (Day Shift, 1.0 FTE) at Providence ElderPlace in Kent, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.
Schedule is Monday - Friday from 8am to 4:30pm! This position will support the Adult Day Health Program at one of our clinic sites in Kent.
In this position you will:
Assist nursing staff in planning and compliance with medication regime for each participant. Administer care based on the nursing process, the interdisciplinary care plan and the nursing care plan. Provides direct participant care, including but not limited to: vital signs, wound care, medication administration/management, treatments, patient teaching, venipuncture and microcoagulation, catheter care foot care, administration/reading PPD tests and patient set up for physician visit.
Initiates a participant education plan according to the individualized needs of the patient, in conjunction with the Primary Care Physician and the Primary Nurse.
Contributes to the total care plan by making pertinent nursing observations and participating in multidisciplinary team care conferences, staff meetings and in-services as needed.
Assures that proper equipment, instruments and supplies are available to meet patient needs.
Assures that documentation reflects adherence to nursing standards, interdisciplinary care plan and progress toward identified patient outcomes.
Required qualifications for this position include:
Graduate of an accredited LPN or LVN program.
One (1) year of long term care or related geriatric experience.
Experience working with the frail and elderly
Current Washington State License as a Practical/Vocational Nurse.
Current CPR certification.
