Providence is calling a Licensed Practical Nurse (LPN) (1.0 FTE, Variable Shift) to work in Transitional Care at Providence Marianwood in Issaquah, WA.

The Licensed Practical Nurse fulfills the Mission of Providence Health & Services in working with residents, families, volunteers, and PMW staff. Nursing care is holistic and is delivered in accordance with the PH&S vision, relevant professional nursing standards, and the Washington State licensed practical nurse scope of practice.

The LPN follows all relevant policies and procedures and works within the Licensed Practical Nurse scope of practice defined in the Revised Code of Washington/Washington Administrative Code. The LPN is a member of the Household Team, providing clinical and health-related care, and participates in all aspects of resident-directed care by participating in non-health-related activities.

The Licensed Practical Nurse at Providence Marianwood applies up-to-date knowledge and skills when providing care in accordance with the resident's individualized care plan. Nursing care is always delivered in partnership with residents and families to ensure balance between health-related recommendations and resident values and preferences.

In this position you will:

  • Accountable for nursing-related resident outcomes during the shift.

  • Oversee the work of the Resident Assistants, and provides direction and feedback to ensure resident needs are met.

  • Participates in all aspects of resident care, including but not limited to assistance with activities of daily living, activities, and food preparation.

  • Document resident health status and care provided in accordance with clinical, ethical, and legal standards.

  • Complete other functions delegated by the primary nurse or Nurse Manager, including but not limited to transcribing and following up on provider orders, ordering medications, completing portions of the move-in or discharge process, completing portions of the MDS as assigned, and updating care plans.

  • Use the nursing process to deliver holistic, high quality nursing care, and is accountable for care outcomes.

  • Use SBAR or another effective communication method to provide accurate information to providers, primary nurse, or supervisors to ensure resident needs are met.

  • Mentor new employees and serves as a preceptor for practical nursing students.

  • Is aware of quality improvement activities and goals, and participate in monitoring and reporting of quality data for the neighborhood; Participate in survey-related activities (including the plan of correction).

  • In collaboration with supervisor, evaluates own practice, identify goals for growth, and identify resources to reach goals.

  • Deliver holistic nursing care in accordance with the resident plan of care.

  • Use the nursing process to assess effectiveness of the current care plan, and update the care plan in collaboration with the Primary Nurse.

Required qualifications for this position include:

  • Graduate of an accredited Licensed Practical Nurse (LPN) program

  • Current Licensed Practical Nurse (LPN) licensure in Washington State

  • Valid Cardiopulmonary Resuscitation (CPR) Certification

Preferred qualifications for this position include:

  • One (1) year experience
