Providence is calling a RN Supervisor Senior Village - Brookside Manor (1.0 FTE, Day) to Brookside Manor Hood River in Hood River, OR

We are seeking a RN Supervisor Senior Village - Brookside Manor that will BE responsible for resident health care services on a 24-hour/7 day week basis. Participates in service planning, as well as renewal and maintenance of these plans. Recommends health care service goals and assists in their implementation. Works with the Executive Director in the coaching, orientation and training of staff. Supervises Medication Aides, and LPN on call. Provides clinical services to residents, implements/maintains delegation process of appropriate cares as outlined by the Oregon State Board of Nursing.

In this position you will have the following responsibilities:

  • Together with the Administration, resident, resident’s family, and caregiver, plan, analyze, and evaluate the need of the resident to formulate the service plan of care. Participate in on-going service plan meeting. Work with the leader to screen applicant prior to move-in to ensure appropriate placement.

  • Perform initial nursing assessment for new resident within first week of admission and monitor ongoing health care need quarterly, or as needed with change, notifying appropriate health care provider as needed. Will take RN call as directed.

  • Participate in development, implementation, review, and necessary modification to health service policies and procedure for resident care services and medication management.

  • Perform on-going staff teaching, delegation of special nursing tasks and medication administration in accordance with the Oregon State Board of Nursing Administrative Rules, Chapter 851, Division 047

  • In conjunction with the Executive Director will oversee and review medication administration and Health Services:

  • Assure accurate transcription of physician orders into treatment/medication record.

  • Review resident service record, treatment, and medication book to assure quality of care.

  • Verify physician order on move-in and continuously thereafter for information and initiate appropriate action/follow-up.

  • Supervise ordering of medications from pharmacy.

  • If indicated document a nursing plan for the resident’s care/service plan including delegation procedure, frequency of nurse follow-up visit, and signature and license number of the RN doing the delegating.

  • Has supervisory responsibility for all medication aide or other staff that administer medication to resident.

  • Assist with overseeing all the staff of Brookside Manor and steps in for disciplinary action as needed.

  • Attend all in-service/meeting and other program for facility specialization and standard. Has 20 hours of documented Division-approved continuing education credit each year.

  • Participates in QI activities

  • Maintain good rapport by interacting in conflict or stressful situations in a manner that facilitate constructive resolution.

  • Provide intermittent nursing service for resident whose medical need are stable and predictable. Coordinate health service with outside service provider such as hospice, home health, physicians’ offices etc. when a resident’s medical condition is complex, unstable, or unpredictable.

  • Communicate effectively and manage conflict in a positive manner.

  • Maintain R.N. to staff communication book with current update of resident changing health need.

  • Perform specific nursing task based upon facility policy.

  • Examine and administer first aid to resident and personnel within scope of practice. Complete incident report and notify physician as appropriate.


Required qualifications for this position includes:

  • Education to meet certification, license or registration requirement.

  • Three (3) years: In home health or geriatric nursing in a similar position.

  • Current unencumbered Oregon RN license through the Oregon State Board of Nursing.

  • CPR Certification

Preferred qualifications for this position include:

  • Rehabilitative and restorative experience.
