Providence Health & Services RN Care Specialist - Transition of Care Support in Everett, Washington
Providence is calling a Registered Nurse Care Specialist to Providence Medical Group in Everett, WA.
This is a full time (Monday - Friday; Day Shift) and benefits eligible position.
Please Note: This RN Care Specialist position is primarily a Phone Triage and Outreach role. The RN will be supporting the managed care team in Transition of Care (TOC) outreach calls for patients who are transitioning from the hospital setting and engaging them as they enter back into the primary care environment. The position will be located either at the main Mill Creek Clinic or the North Everett Clinic in Snohomish County.
In this position you will:
Have responsibility for managing patient populations with chronic illness and/or multiple co-morbidities.
Use skilled assessment, education, collaboration and coordination of healthcare and community resources to perform role.
Assist patients/families, staff and systems to achieve high quality, evidence based, cost-effective, and patient focused outcomes.
May perform other ambulatory care RN duties.
Required qualifications for this position include:
Graduate of an accredited school of nursing
Current Registered Nurse (RN) licensure in Washington State
Minimum 2 years RN experience
Preferred qualifications for this position include:
Bachelor of Science Degree in Nursing (BSN)
Certification of diabetes education (CDE)
Experience managing complex patient needs in an ambulatory setting
Experience with Medical Home Model
