Providence is calling an Patient Care Navigator (1.0 FTE, Day Shift) to work in the Cancer Center Partnership at Providence Regional Medical Center Everett in Everett, WA.

Serves as the Cancer Program's link to support services for patients, physicians, and staff alike and includes clinical knowledge relevant to housing, transportation, public benefits, medical legal advocacy, and other related resources. Supports patients, physicians, and staff by identifying and connecting patients with these resources within both the community and Providence health system alike.Responsible for establishing beneficial relationships within the healthcare community and the host facility.
In this position you will:
Acts as liaison between the patient and the various clinic areas, hospitals, and ancillary services to coordinate activities involved for patient care
Contacts and assists patients who are “at risk” for barriers to care, in coordination with and as directed by case manager and/or care team.
Participates in multidisciplinary planning conferences.
Promotes and protects general safety for patients/families/friends and co-workers through adherence to correct procedures and timely communication of messages.
Communicate with health care team members regarding the status and/or completion of care coordination activities.
Speak with constituents in private to assess both immediate or through appointment system and help provide access to the services and support they require
Required qualifications for this position include:
Bachelor's Degree in Social Work
2 years helath related field experience
2 years experience in a patient care setting involving interaction with a variety of medical services, contact with patients and families, and interaction with physicians and providers.
3 years experience in providing a high level of customer service including handling confidential patient health information (PHI) with discretion.
Preferred qualifications for this position include:
Social services focused background.
Electronic Medical Records (EMR) experience.
Social Service Certification
CCM certification is a plus.
