We are seeking a Medication Reconciliation Pharmacy Technician who will perform functions associated with reconciliation of outpatient prescriptions (prior to admission medication list review) and other hospital pharmacy processing which are permitted by state regulations under the supervision of a licensed pharmacist. Performs routine functions assisting the pharmacist in collecting and verifying patient medication histories

In this position you will:

  • Assist the pharmacist by performing the following medication history activities, with the goal of compiling an accurate and complete medication list prior to hospital admission

  • Collection and clarification of the home medication list through patient or caregiver interview, information gathered from electronic sources, community pharmacies and primary care provider.

  • Verification of allergy information.

  • Entry of the home medication list into the electronic health record (EHR), as well as updating the patient’s preferred community pharmacy information in the EHR

  • Documentation in the EHR and communication of relevant information associated with home medications in the EHR and/or verbally to the pharmacist.

  • Communicating concisely and effectively with patients, families, pharmacists, and other health care providers related to patient home medication histories.

  • Demonstrate age specific patient care considerations for the population served

  • Demonstrate application of knowledge, skills, and behaviors in the physical, motor/sensory, cognitive, psychosocial, patient and family education areas based on PRMCE age specific care competency assessment tool

Qualifications:

Required qualifications for this position include:

  • Registered with WA State Board of Pharmacy as a Pharmacy Technician

Preferred qualifications for this position include:

  • Third party billing or reimbursement

  • Prior medication list reconciliation experience
