Medical Social Worker - Home Health Snohomish at Home Health Snohomish

This position is responsible for providing referral services to home care clients, families and caregivers, addressing issues such as adjustment to illness/prognosis, care system issues, community resource issues, and insurance/financial issues. The MSW develops and implements a plan of care in collaboration with the Home Care team providing care to the patient.

Shift: Day

FTE/ Employment Status: 1.0 (Full-Time)

Location: Snohomish County

Required qualifications:

  • Master's Degree of Social Work

  • Current WA Registered Counselor or Social Worker Licensure

  • Current CPR certification.

  • Valid Washington State driver’s license and have access to daily use of an insured reliable vehicle or an alternate ability to travel to sites

  • 1 year health care setting.

Preferred Qualifications:

  • 2 years health care setting.

  • Home health care setting.
